| Job Description: | Provides direct and indirect interventions to predialysis and chronic dialysis patients served by the NC Health Systems Management Centers. The Social Worker provides clinical services in collaboration with the multidisciplinary health care team in order to assist patients in reaching their fullest rehabilitative potential. |
| Qualifications: | Masters degree in Social Work from a graduate school of social work accredited by the Council of Social Work Education and at least one year of experience in medical or psychiatric social work. |
